Is Antihypertensives (Multiple Classes) Safe with Citrulline?

Inform your physician before starting citrulline supplementation if you take any blood pressure medications. Monitor blood pressure closely, especially during exercise. Start with the lowest effective citrulline dose (2-3g vs typical 6-8g). Avoid exercising in the heat until blood pressure response is established.
Why this interaction occurs
Citrulline significantly increases nitric oxide bioavailability, producing dose-dependent reductions in systolic blood pressure of 4-8 mmHg in clinical studies. This effect is additive with all antihypertensive drug classes including ACE inhibitors, ARBs, calcium channel blockers, and diuretics. In patients whose blood pressure is already well-controlled, this combination during vigorous exercise can cause symptomatic hypotension, syncope, and falls. The effect is most pronounced with doses above 6g/day.
Evidence
Evidence level: strong.
Reported safety data
The FDA’s adverse event reporting system (FAERS) has received 639 reports mentioning Citrulline. This reflects reported cases involving the medication in general — not confirmed causation, and not specific to the Citrulline + supplement combination on this page.
FAERS reports are voluntary and cannot prove that a drug caused an event. Do not use report counts to assess the safety of this specific drug-supplement pair.
