WHAT YOU NEED TO KNOW

Bacillus coagulans may be taken with clindamycin due to its spore-forming nature providing partial resistance; however, separate doses by at least 2 hours to optimize survival. Continue supplementation throughout and for 2–4 weeks after antibiotic therapy. Bacillus coagulans is a preferred probiotic choice during antibiotic therapy for this reason.

Why this interaction occurs

Unlike vegetative cell-based probiotics, Bacillus coagulans forms heat- and antibiotic-resistant endospores, conferring partial but not complete resistance to clindamycin. Studies show better survival rates of spore-forming Bacillus strains during antibiotic therapy compared to Lactobacillus or Bifidobacterium. However, some reduction in viable spore germination may still occur, warranting dose separation.

Evidence

Evidence level: moderate.

Reported safety data

The FDA’s adverse event reporting system (FAERS) has received 42,468 reports mentioning Clindamycin. This reflects reported cases involving the medication in general — not confirmed causation, and not specific to the Clindamycin + supplement combination on this page.

FAERS reports are voluntary and cannot prove that a drug caused an event. Do not use report counts to assess the safety of this specific drug-supplement pair.
