Avoid combining berberine with semaglutide unless under close medical supervision. Monitor blood glucose frequently. Watch for symptoms of hypoglycemia including dizziness, sweating, and confusion. Dose adjustment may be required.
Why this interaction occurs
Berberine activates AMPK and inhibits intestinal alpha-glucosidase, producing insulin-sensitizing and glucose-lowering effects that are mechanistically additive to semaglutide's GLP-1 receptor agonism. Combined use significantly increases hypoglycemia risk, particularly in patients with type 2 diabetes also on insulin or sulfonylureas.
Evidence
Evidence level: moderate.
