Is Calcium Safe with Metformin?

Take calcium carbonate or calcium citrate (500-1000 mg/day) as directed. Note that calcium supplementation may partially restore metformin-impaired B12 absorption. Take calcium with vitamin D for optimal absorption. Avoid taking calcium at the same time as metformin.
Why this interaction occurs
Metformin reduces luminal calcium bioavailability in the terminal ileum, which directly disrupts the calcium-dependent uptake of the intrinsic factor-B12 complex. Studies show that calcium supplementation (1.2 g/day) can partially reverse metformin-associated B12 malabsorption, creating an important downstream link in the nutrient depletion cascade. Long-term metformin use may also independently reduce bone mineral density.
Evidence
Evidence level: moderate.
Reported safety data
The FDA’s adverse event reporting system (FAERS) has received 432,898 reports mentioning Metformin. This reflects reported cases involving the medication in general — not confirmed causation, and not specific to the Metformin + supplement combination on this page.
FAERS reports are voluntary and cannot prove that a drug caused an event. Do not use report counts to assess the safety of this specific drug-supplement pair.
