Is Folate (Folic Acid / Methylfolate) Safe with Metformin?

Supplement with 400-800 mcg of methylfolate (5-MTHF) daily, especially in women of childbearing age. Request homocysteine testing annually. Eat folate-rich foods (leafy greens, legumes) regularly.
Why this interaction occurs
Metformin reduces folate absorption through alterations in intestinal transport and gut microbiome changes. Folate depletion elevates plasma homocysteine levels, increasing cardiovascular and cerebrovascular risk. In pregnant or pre-conception patients, folate deficiency raises neural tube defect risk significantly. Methylfolate is preferred over folic acid for patients with MTHFR polymorphisms, which are common in type 2 diabetics.
Evidence
Evidence level: moderate.
