Is Folate (Methylfolate) Safe with Glucophage?

Supplement with 400-800 mcg methylfolate daily. Avoid high-dose folic acid (>1000 mcg) without medical guidance. Request homocysteine blood testing at baseline and annually. Women of childbearing potential should prioritize folate sufficiency before and during pregnancy.
Why this interaction occurs
Glucophage reduces serum folate levels through impaired intestinal absorption and altered folate metabolism. Depleted folate impairs the remethylation of homocysteine to methionine, causing hyperhomocysteinemia, which is an independent cardiovascular risk factor particularly dangerous in diabetic patients who already have elevated cardiac risk. Methylfolate (5-MTHF) bypasses MTHFR enzyme conversion issues common in diabetic populations and is the preferred supplemental form.
Evidence
Evidence level: moderate.
Reported safety data
The FDA’s adverse event reporting system (FAERS) has received 38,342 reports mentioning Glucophage. This reflects reported cases involving the medication in general — not confirmed causation, and not specific to the Glucophage + supplement combination on this page.
FAERS reports are voluntary and cannot prove that a drug caused an event. Do not use report counts to assess the safety of this specific drug-supplement pair.
