Is Folate (Vitamin B9) Safe with Aripiprazole?
Free · No account · based on published sources including NIH ODS, FDA drug labels, and peer-reviewed literature
Folate supplementation (400–800 mcg/day of methylfolate preferred in patients with MTHFR variants) is generally safe with aripiprazole. Discuss with your psychiatrist if you have a history of cognitive symptoms, depression, or elevated homocysteine. It is a low-risk adjunct.
Why this interaction occurs
Folate deficiency is prevalent in patients with schizophrenia and other psychotic disorders and is associated with elevated homocysteine, impaired one-carbon metabolism, and worsened negative and cognitive symptoms. L-methylfolate has been studied as an adjunct to antipsychotics including aripiprazole, with evidence suggesting improvements in negative symptoms particularly in patients with the MTHFR C677T polymorphism. No pharmacokinetic interaction with aripiprazole has been identified.
Evidence
Evidence level: moderate.
Reported safety data
The FDA’s adverse event reporting system (FAERS) has received 42,796 reports mentioning Aripiprazole. This reflects reported cases involving the medication in general — not confirmed causation, and not specific to the Aripiprazole + supplement combination on this page.
FAERS reports are voluntary and cannot prove that a drug caused an event. Do not use report counts to assess the safety of this specific drug-supplement pair.
