Avoid concurrent use when possible. If used together, start FOS at the lowest possible dose (≤2g/day) and titrate slowly. Advise patients of increased flatulence, bloating, and abdominal cramping.
Why this interaction occurs
Acarbose inhibits intestinal alpha-glucosidases, causing undigested carbohydrates to reach the colon where bacteria ferment them, producing gas. FOS (fructooligosaccharides) are rapidly fermented by colonic bacteria, also producing significant gas. Combined use leads to substantially increased colonic fermentation, causing additive flatulence, bloating, and diarrhea that may be intolerable for patients.

Evidence level: strong.
Reported safety data
The FDA’s adverse event reporting system (FAERS) has received 4,031 reports mentioning Acarbose. This reflects reported cases involving the medication in general — not confirmed causation, and not specific to the Acarbose + supplement combination on this page.
FAERS reports are voluntary and cannot prove that a drug caused an event. Do not use report counts to assess the safety of this specific drug-supplement pair.
