Is Magnesium Safe with Furosemide?

Have serum magnesium levels checked regularly. Discuss magnesium supplementation with your doctor, especially if experiencing muscle cramps, tremors, or cardiac symptoms. Do not exceed recommended doses without supervision.
Why this interaction occurs
Furosemide increases urinary magnesium excretion by inhibiting tubular reabsorption. Hypomagnesemia can worsen hypokalemia (making it refractory to potassium replacement), cause neuromuscular irritability, and precipitate cardiac arrhythmias. Concurrent magnesium depletion is a major reason potassium replacement fails in loop diuretic patients.
Evidence
Evidence level: strong.
