Avoid all cinnabar-containing TCM products (e.g., An Gong Niu Huang Wan, Zhu Sha An Shen Wan). If already in use, obtain urinary mercury levels immediately. Discontinue and seek medical evaluation for tremor, cognitive changes, or renal dysfunction.
Why this interaction occurs
Cinnabar (HgS) is used in several classical TCM formulations as a sedative. Although inorganic mercury sulfide has lower bioavailability than organic mercury, gastrointestinal bacteria can convert it to more toxic forms. Chronic exposure causes nephrotoxicity, neurotoxicity, and acrodynia. Several regulatory agencies including Health Canada and the EU have banned these products.
Evidence
Evidence level: strong.
