Is Spermidine Safe with Sirolimus?

Use with caution in transplant patients on sirolimus. Discuss with your specialist as the combination may unpredictably alter immunosuppressive effect.
Why this interaction occurs
Spermidine exerts its anti-aging effects partly through mTOR inhibition and autophagy induction — the same primary mechanism by which sirolimus exerts immunosuppression. Concurrent use may produce additive mTOR suppression leading to over-immunosuppression, increased infection susceptibility, or impaired wound healing, while potentially altering the predictability of sirolimus drug monitoring.
Evidence
Evidence level: limited.
Reported safety data
The FDA’s adverse event reporting system (FAERS) has received 14,104 reports mentioning Sirolimus. This reflects reported cases involving the medication in general — not confirmed causation, and not specific to the Sirolimus + supplement combination on this page.
FAERS reports are voluntary and cannot prove that a drug caused an event. Do not use report counts to assess the safety of this specific drug-supplement pair.
