Is Vitamin B12 (Cyanocobalamin) Safe with Pantoprazole?
Free · No account · based on published sources including NIH ODS, FDA drug labels, and peer-reviewed literature
Patients on pantoprazole for more than 2 years should monitor serum B12 levels annually. Consider sublingual or intramuscular B12 supplementation, which bypasses the gastric acid-dependent absorption pathway.
Why this interaction occurs
Vitamin B12 absorption requires pepsin and gastric acid to cleave it from dietary protein, and intrinsic factor for ileal uptake. Pantoprazole suppresses acid production, reducing pepsin activity and impairing B12 release from food. Crystalline B12 supplements are better absorbed than food-bound B12 in this context, but absorption is still reduced compared to normal.
Evidence
Evidence level: strong.
Reported safety data
The FDA’s adverse event reporting system (FAERS) has received 267,421 reports mentioning Pantoprazole. This reflects reported cases involving the medication in general — not confirmed causation, and not specific to the Pantoprazole + supplement combination on this page.
FAERS reports are voluntary and cannot prove that a drug caused an event. Do not use report counts to assess the safety of this specific drug-supplement pair.
