WHAT YOU NEED TO KNOW

Do not self-initiate Acetyl L-Carnitine during cisplatin chemotherapy. Discuss with your oncologist, as timing relative to chemotherapy cycles is critical.

Why this interaction occurs

Cisplatin causes peripheral neuropathy partly through mitochondrial damage in neurons. Acetyl L-Carnitine has shown neuroprotective benefit in some trials. However, its antioxidant and mitochondria-stabilizing properties raise concern that it could also protect cancer cells from cisplatin-induced oxidative cytotoxicity. A large RCT (Hershman et al.) showed no neuropathy benefit and a trend toward worse outcomes in breast cancer, warranting caution.

Evidence

Evidence level: strong.

Reported safety data

The FDA’s adverse event reporting system (FAERS) has received 80,762 reports mentioning Cisplatin. This reflects reported cases involving the medication in general — not confirmed causation, and not specific to the Cisplatin + supplement combination on this page.

FAERS reports are voluntary and cannot prove that a drug caused an event. Do not use report counts to assess the safety of this specific drug-supplement pair.
