Is Magnesium Safe with Quetiapine?

Avoid high-dose magnesium supplementation without medical supervision. Routine dietary magnesium intake is generally safe. If magnesium is prescribed therapeutically, inform your cardiologist and psychiatrist so ECG monitoring can be arranged.
Why this interaction occurs
Quetiapine prolongs the QTc interval through hERG potassium channel blockade. Magnesium plays a critical role in cardiac electrophysiology and membrane stabilization. While moderate magnesium supplementation may actually be cardioprotective, hypomagnesemia (which can result from inadequate intake or diuretic use) significantly worsens quetiapine-induced QTc prolongation and increases torsades de pointes risk. Electrolyte balance must be carefully maintained.
Evidence
Evidence level: moderate.
Reported safety data
The FDA’s adverse event reporting system (FAERS) has received 111,480 reports mentioning Quetiapine. This reflects reported cases involving the medication in general — not confirmed causation, and not specific to the Quetiapine + supplement combination on this page.
