Is Melatonin Safe with Quetiapine?

Avoid taking melatonin concurrently with quetiapine unless advised by your doctor. If used, take melatonin at the lowest effective dose (0.5–1 mg) and monitor for excessive drowsiness, dizziness, or difficulty waking.
Why this interaction occurs
Quetiapine has significant sedative and hypnotic properties mediated through H1 receptor antagonism. Melatonin also promotes sleep onset via MT1/MT2 receptor agonism. Concurrent use produces additive CNS depression, increasing risk of excessive sedation, psychomotor impairment, and next-day cognitive dulling. This combination can also compound orthostatic hypotension risk.
