WHAT YOU NEED TO KNOW

Monitor serum magnesium levels every 3–6 months in patients on long-term esomeprazole. Supplement with magnesium citrate or glycinate (better absorbed forms) if hypomagnesemia is detected. Be aware that serum magnesium may not reflect total body stores accurately.

Why this interaction occurs

Chronic esomeprazole use (typically >1 year) inhibits TRPM6/TRPM7 magnesium transporters in the intestinal epithelium, reducing active magnesium absorption. This can result in hypomagnesemia even when dietary intake is adequate. Low magnesium can cause muscle cramps, cardiac arrhythmias, and secondary hypocalcemia and hypokalemia.

Evidence

Evidence level: strong.

Reported safety data

The FDA’s adverse event reporting system (FAERS) has received 70,713 reports mentioning Esomeprazole. This reflects reported cases involving the medication in general — not confirmed causation, and not specific to the Esomeprazole + supplement combination on this page.

FAERS reports are voluntary and cannot prove that a drug caused an event. Do not use report counts to assess the safety of this specific drug-supplement pair.
