Is Vitamin C (Ascorbic Acid) Safe with Esomeprazole?

Vitamin C supplementation can be used strategically alongside esomeprazole to enhance iron absorption. No dose adjustment of vitamin C is required, but patients should be aware that gastric vitamin C concentrations are altered by PPI therapy, which may have implications for gastric mucosal protection.
Why this interaction occurs
Esomeprazole raises gastric pH, which alters the oxidation-reduction balance of ascorbic acid in the stomach. Under normal acid conditions, vitamin C exists in its reduced (active) form; under hypochlorhydric conditions, it may be more readily oxidized to dehydroascorbic acid. Additionally, H. pylori-associated gastritis (sometimes co-existing with PPI use) further depletes gastric vitamin C. The interaction is low severity but clinically relevant as a compensatory strategy for iron absorption.
Evidence
Evidence level: moderate.
Reported safety data
The FDA’s adverse event reporting system (FAERS) has received 70,713 reports mentioning Esomeprazole. This reflects reported cases involving the medication in general — not confirmed causation, and not specific to the Esomeprazole + supplement combination on this page.
FAERS reports are voluntary and cannot prove that a drug caused an event. Do not use report counts to assess the safety of this specific drug-supplement pair.
