Consider selenium supplementation at 55–200 mcg daily as selenomethionine. Monitor thyroid function (TSH, free T3) in patients with pre-existing thyroid conditions. Do not exceed 400 mcg/day due to toxicity risk.
Why this interaction occurs
Ozempic-induced dietary restriction reduces intake of selenium-rich foods including Brazil nuts, seafood, and organ meats. Selenium is an essential cofactor for deiodinase enzymes responsible for converting T4 to active T3. Depletion may worsen thyroid function and impair antioxidant defense via glutathione peroxidase, relevant in patients with autoimmune thyroid disease.
