Is Vitamin B12 (Methylcobalamin) Safe with Rabeprazole?

For patients on rabeprazole longer than 2 years, screen serum B12 levels annually. Prefer sublingual methylcobalamin or intramuscular cyanocobalamin injections over oral tablets for supplementation, as these bypass acid-dependent gastric absorption steps. Target serum B12 above 300 pg/mL to avoid neurological complications.
Why this interaction occurs
Rabeprazole suppresses gastric acid and pepsin secretion, impairing proteolytic release of B12 from dietary protein-bound complexes. Although intrinsic factor secretion from parietal cells is maintained, inadequate free B12 results in reduced ileal absorption. Long-term deficiency leads to megaloblastic anemia and subacute combined degeneration of the spinal cord. Risk is higher in elderly patients, vegetarians, and those with pre-existing low B12 levels.
Evidence
Evidence level: strong.
Reported safety data
The FDA’s adverse event reporting system (FAERS) has received 24,791 reports mentioning Rabeprazole. This reflects reported cases involving the medication in general — not confirmed causation, and not specific to the Rabeprazole + supplement combination on this page.
FAERS reports are voluntary and cannot prove that a drug caused an event. Do not use report counts to assess the safety of this specific drug-supplement pair.
