Is CoQ10 (Ubiquinone) Safe with Rosuvastatin?
Free · No account · based on published sources including NIH ODS, FDA drug labels, and peer-reviewed literature
If experiencing muscle aches, fatigue, exercise intolerance, or weakness on rosuvastatin, ask your pharmacist or physician about CoQ10 supplementation (100–300 mg/day of ubiquinol or ubiquinone form). Do not stop rosuvastatin without medical advice.
Why this interaction occurs
Rosuvastatin, like all statins, inhibits HMG-CoA reductase and thereby depletes endogenous CoQ10 by interrupting the mevalonate pathway. CoQ10 is essential for mitochondrial electron transport chain function and ATP production in muscle cells. Depletion may contribute to statin-associated muscle symptoms (SAMS). Clinical trials show variable but measurable reductions in plasma CoQ10 with rosuvastatin use, and CoQ10 supplementation has shown benefit in some SAMS patients.
Evidence
Evidence level: moderate.
Reported safety data
The FDA’s adverse event reporting system (FAERS) has received 115,467 reports mentioning Rosuvastatin. This reflects reported cases involving the medication in general — not confirmed causation, and not specific to the Rosuvastatin + supplement combination on this page.
FAERS reports are voluntary and cannot prove that a drug caused an event. Do not use report counts to assess the safety of this specific drug-supplement pair.
