Why this interaction occurs
Both donepezil and Huperzine A inhibit acetylcholinesterase, the enzyme that breaks down acetylcholine. Concurrent use causes additive to synergistic cholinergic toxicity, risking bradycardia, excessive secretions, muscle fasciculations, nausea, vomiting, seizures, and respiratory depression.
