Is Melatonin Safe with Citalopram?

Patients experiencing SSRI-related insomnia or sleep disruption may consider low-dose melatonin (0.5–3 mg, 30 minutes before bedtime). Use the lowest effective dose. Inform prescriber as melatonin may interact with serotonergic pathways.
Why this interaction occurs
SSRIs including citalopram can suppress endogenous melatonin synthesis by upregulating serotonin availability, which paradoxically inhibits pineal melatonin production via 5-HT2C receptor activation. This results in disrupted circadian rhythm and insomnia, a common side effect. Low-dose exogenous melatonin may partially restore sleep architecture without significant pharmacokinetic interaction with citalopram.

Evidence level: moderate.
Reported safety data
The FDA’s adverse event reporting system (FAERS) has received 113,877 reports mentioning Citalopram. This reflects reported cases involving the medication in general — not confirmed causation, and not specific to the Citalopram + supplement combination on this page.
FAERS reports are voluntary and cannot prove that a drug caused an event. Do not use report counts to assess the safety of this specific drug-supplement pair.
