Is Potassium Safe with Hydrochlorothiazide?

Have potassium levels monitored at baseline and periodically. Potassium-rich foods or supplementation may be recommended by your provider. Never self-adjust potassium supplementation as both hypo- and hyperkalemia are dangerous.
Why this interaction occurs
Hydrochlorothiazide inhibits the Na-Cl cotransporter in the distal convoluted tubule, increasing sodium and water delivery to the collecting duct, which stimulates aldosterone-mediated potassium secretion. This leads to dose-dependent urinary potassium wasting and hypokalemia, increasing risk of arrhythmias and glucose intolerance.
Evidence
Evidence level: strong.
Reported safety data
The FDA’s adverse event reporting system (FAERS) has received 215,975 reports mentioning Hydrochlorothiazide. This reflects reported cases involving the medication in general — not confirmed causation, and not specific to the Hydrochlorothiazide + supplement combination on this page.
FAERS reports are voluntary and cannot prove that a drug caused an event. Do not use report counts to assess the safety of this specific drug-supplement pair.
