Is ACE2/Angiotensin 1-7 (oral bioencapsulated supplement) Safe with Enalapril?
Free · No account · based on published sources including NIH ODS, FDA drug labels, and peer-reviewed literature
Patients taking enalapril or other ACE inhibitors should be aware that these drugs inhibit ACE2 enzyme activity (>90% inhibition at higher doses in vitro), significantly reducing the benefit of oral ACE2/Ang1-7 supplementation; if ACE2 supplementation is intended, consider discussing ARB-based therapy with a physician as ARBs do not inhibit ACE2.
Why this interaction occurs
In vitro and in vivo data in MMVD dogs demonstrated that ACEIs (enalapril, benazeprilat, lisinopril) inhibit both soluble and membrane-bound ACE2 activity, with lisinopril inhibiting at much lower doses. ACEI background therapy also reduced the Ang1-7 pool proportionately, counteracting the intended mechanism of oral ACE2/Ang1-7 supplementation. ARBs did not share this inhibitory effect and produced 670-755% greater increases in sACE2 activity with the supplement.

Reported safety data
The FDA’s adverse event reporting system (FAERS) has received 61,312 reports mentioning Enalapril. This reflects reported cases involving the medication in general — not confirmed causation, and not specific to the Enalapril + supplement combination on this page.
FAERS reports are voluntary and cannot prove that a drug caused an event. Do not use report counts to assess the safety of this specific drug-supplement pair.
