Is Magnesium Safe with Tacrolimus?
Free · No account · based on published sources including NIH ODS, FDA drug labels, and peer-reviewed literature
Magnesium supplementation is often required with tacrolimus but must be physician-guided. Use oral magnesium oxide or glycinate as directed. Avoid intravenous self-administration. Monitor renal function and magnesium levels monthly.
Why this interaction occurs
Tacrolimus induces significant renal magnesium wasting through inhibition of TRPM6 channels in the distal tubule. While supplementation is clinically indicated, excessive or unmonitored magnesium use in patients with CKD or post-transplant kidney dysfunction can lead to hypermagnesemia due to reduced renal clearance, causing neuromuscular and cardiovascular toxicity.
Evidence
Evidence level: moderate.
