Is Magnesium Safe with Fluconazole?
Free · No account · based on published sources including NIH ODS, FDA drug labels, and peer-reviewed literature
Monitor magnesium levels before and during fluconazole therapy. Do not take high-dose magnesium supplements without physician approval. Seek immediate care if you experience palpitations, dizziness, or fainting.
Why this interaction occurs
Fluconazole inhibits CYP2C9 and CYP3A4 and independently prolongs the QT interval. Hypomagnesemia further potentiates QT prolongation, increasing the risk of serious ventricular arrhythmias including Torsades de Pointes. Conversely, excessive magnesium supplementation without monitoring can unpredictably affect cardiac conduction.
Evidence
Evidence level: strong.
